Four times, actually, since you include the Minion Reflector. The original creature will trigger twice, as will the token copies of it.

Hmmmh, There are no creatures in Play. I cast a ... Cathodion and pay 2 for the Reflector to get a token copy.
Then I sacrifice the original Cathodion triggering the lifeline and its ltb ability (1). Then I sacrifice the token to trigger its ltb ability (2).
At end of turn the oríginal Cathodion comes back and I may pay for the token generator again. Next turn I start with 1 Cathodion (original) and 1 token.
A token won't return by lifeline since it ceases to exists. Did I miss something or are the ltb Triggers limited to 2?

The second time the creature enters, Minion Reflector triggers again, giving you a second token.

I really wish whatever was going on would get fixed and we could edit our posts again.
Anyway:
-Original creature enters
-Reflected copy enters
-sac original
-original re-enters
-second reflector copy enters.
So that is four entry triggers. As for death triggers, you do lose out on them, having to jump through the hoop of Lifeline requiring a creature when it checks to return the dead creature.

I was talking about the ltb triggers. Anyway, terakhan explained the best case of triggers.

At most I have 2 of them during my main phases.

I have at most 4 of them the turn I cast my original creature first or I sacrifice both creatures during main phase and during end step.

If I sacrifice a creature during end step, then the max ltb trigger Count of the following turn is decreased by the number of creatures n I sacrifice (n equals either 1 or 2).

If I play it like described above I'll have 2 Triggers each turn since I can sacrifice the original creature triggering lifeline and
