Needs 9 mana, hopefully you have Chromatic lantern in play along with some mana rocks so the combo can go off earlier. 
Play Scouting Trek, then Traumatize yourself, then Planar Birth.  you may lose some other cards, depending on the number of basics you run and the turn you can pull it off.
Plenty of ways to stop it, but fun if you can do it.

Replace the Traumatize with Countryside Crusher, so you get a beefy creature and dont lose any non-lands to the trick.
