EDIT: My apologies, RS420 already posted a Sutured Ghoul/Shallow Grave combo.
Note: You only need the two Rituals, the Buried alive, the Shallow Grave, and the Swamp in your hand, not the three creatures.
1:Drop the Swamp
2:Tap it to play the Dark Rituals(5 black mana)
3:Play Buried Alive, sending Phyrexian Dreadnought, Death's Shadow, and Sutured Ghoul(on top of the others) to the graveyard. (2 black mana left in mana pool)
4:Cast Shallow Grave, pulling the Sutured Ghoul onto the battlefield, exiling the Dreadnought and Shadow. Behold your 25/25.
5.Swing for 25 trample damage.
One can also swap a Cabal Ritual in for one of the DRs and use Reanimate instead of Shallow Grave to bring the Ghoul into play without haste (and exile). The perks to this are that you'll have a higher chance of pulling two different rituals as supposed to two of the same.

How are you supposed to play two Dark Rituals with one swamp?

2 dark rituals is easy to play with 1 swamp...
tap swamp for 1 black, cast 1 dark ritual, now you have 3 black mana, use 1 black mana for another dark ritual.. now you have 5 black mana
