Simple get all 4 cards in play. Then sacrifice both redcap and runner for 2 mana from altar (lets say green). Then put redcap back in play dealing damage to opponent. Now use the two green to play medicine runners. This will pull the-1 -1 counter u just gave redcap. Rinse and repeat for infinite damage.

The Redcap is a very nasty creature. I think 3 Cards and 1 Color (mono black) is the best way to use it so far. Anybody a better combination?
