Have the 3 enchantments in play: megrim, pain magnification and furnace of rath, then play duress. Furnace of rath will doubble the damage done by megrim, so that megrim as a singel source would deal 4 damage and proc the pain magnifications ability. Then the loop starts. They discard their entire hand for 1 mana duress and loose 4 life for each card.

If it's a big game does every opponent's hand get discarded? It definitely takes some setup so it's probably best to cast Megrim last so they don't know what's coming!

Nope it only works on 1 player. In that case I'd suggest Liliana's Caress to make it go a bit faster.

liliana of the veil's +1 ability would be better than duress.
