have the altar out, or any other sac engine, have the runner out, give the runner undying, sac it, and when it comes back it can take the +1/+1 counter off of itself and endless sac

my question is, will this work?

I want to say no...because a similar issue came up with tatterkite and persist...does the creature retain undying even after re-entering the battlefield? I'm not precisely sure...

the runner gets undying for the entire turn, also creatures with undying don't loose undying when they come back, its like persist.

No.  Once it leaves and reenters the battlefield, it is a whole separate entity with no "memory" of what it once was.  Replace with Mikaeus?

No, this wouldn't work. Once it returns, it loses the instance of the ability.

To clarify, that wasn't a 'no' to Mikaeus, he will allow the combo to work if he's put in place of the Undying Evil.  He will have no memory of his previous existence, but will have Undying because he's on the battlefield with Mikaeus.
