Jace is a proxy for Tamiyo, the Moon Sage

Not a combo so much as a synergy. Get Tamiyo to her ultimate and get the emblem, then start casting whatever you want (for example, Cruel Ultimatum). If you\\\'re discarding to cast an instant or sorcery, then you can go infinite.

With Conflux this becomes a "Get-whatever-you-want-out-of-your-library-at-sorcery-speed-for-free" combo

Doesn't Tamiyo cost 5 and her ultimate cost 8 counters?

Yeah, I have trouble getting into any combo that involves keeping a planeswalker around that long. Very nice use of dream halls, but seriously you'll need a way to get Tamiyo up there other than her crappy +1 ability, Gilder Bairn perhaps?

I'm with you Lin, other than limited formats, Planeswalkers really don't stick around long enough for their ultimate. The only time I see ultimates go off is when there's a doubling season in play so they use the ultimate the turn they come out.

I keep Agent of Bolas around in my UB control deck by stalling on counter magic and removal.

I don't know what you mean by 'crappy +1 ability'.  Tamiyo's first ability is her best (aside from the ultimate)  It protects her by making sure some guys stay out of combat in addition to ticking up her counters.  Usually the minus ability is the one that protects the 'walker, look at Liliana of the Veil.  I play Tamiyo in my FogWalkers deck and she's a house.  Yes the Ultimate can win the game, but if they ability that gets you there isn't good, the walker usually has troubles, this is not her problem.
