I'm going to go ahead and say that this isn't really a combo, just because it's really only one card - tooth and nail.
The Mountian is Tyrant of Discord from Avacyn Restored.
Entwined Tooth fetches the Tyrant and the Razer. Stack their triggered ETB effects so Razer's resolves first. Exile all lands, then Tyrant will destroy all of your opponent's permanents, even indestructible ones. You won't have any lands either, but you'll have a 7/7 and a 4/2 you can beat down with so you should win then.

nice, very very nice :). Seems to be a strong EDH combo, just wonder who would be the best general to tutor it up ;)

This would work best with haste and huge nasty creatures. Eldrazi that have annihilator perhaps with concordant crossroads in play, or maybe tramplers or infect. They'd all work nicely.

Well you really shouldn't need any other creatures than the two. You'll be dealing 11 damage a turn, and unless they can recover really quickly from an destroyed board (which is unlikely), it'll be over in just two turns.
And I'd actually prefer Hazezon Tamar, just because it tends to have more of a ramping element than Mayael which helps with the tooth and nail.
