Swamp is Gravecrawler
Underground Sea is Diregraf Captain

With Phyrexian Altar and Diregraf Captain out you can cast the Gravecrawler, sac it for a black mana, your opponent loses 2 life to the Captain.  Use the black mana to recast it from the graveyard, rinse repeat till everyone else is dead.

I couldn't tell if this combo had been posted before because the cards aren't in the database.  It's similar to the 'no library' combo that was posted recently, but requires 1 fewer card.  Are there any good alternatives to Phyrexian Altar that are legal in Standard or Modern?  I can't think of any.

Gravecrawler is pretty darn good. A 2/1 for B on its own would be decent, however being able to cast it from your graveyard is a huge boon. Look how many combos Crucible of Worlds spawned. Now we have both Gravecrawler and Havengul Lich.

Would be even better if we had them in the database.

Graftdiggers cage stops this and it only cost one mana to cast.

Thats true, just as cards like Torpor Orb stop a lot of combos too. Thats why many people sideboard a copy or two in standard decks. But all combos can be stopped, and it doesn't take much in standard to remove an artifact.
