With Triskelion, Mephidross, and Rite of Passage in play:

Use Triskelion's ability to deal damage to it self, triggering both Mephidross and Rite of Passage triggered abilty, Triskelion lose 1 counter but it gains 2, repeating the process making Triskelion full of counters, thats damages to infinity..

It also works with One Triskelion and Two Mephidross,

Or One Triskelion and Two Rite of Passage,

And also Mephidross with Triskelion in play makes them Creature killing machine, except for indestructibles,

Tip: Tooth and Nail is a big help.

mmm, me likes. One thing I might change is to use Doubling Season instead of Mephidross Vampire (or in addition to it). Using that would allow you to keep it monocolored and letting you play it one turn earlier. This way, with green mana accel, you can drop Rite of Passage on T2, a blocker like Kitchen Finks on T3, Doubling Season on T4, and Triskelion on T5.

This doesn't work. While Triskelion will get the +1/+1 counters, it's not a replacement effect, so it will still take the damage. The first time it's dealt damage, it will die as a state-based effect before the counter is put on.

triskelion enters with 3 counters (3/3), you remove one counter (2/2) to do one damage, making it a 2/2 with 1 damage marked on it.  It then gets the 2 additional counters (4/4 with 1 damage marked) and just gets larger
so yes it works
