forest = inquisitor's flail.   Question: will Phytohydra's ability prevent deathtouch or wither/infect?

for deathtouch, yes because that ability has to deal damage to activate and since the hydra's effect replaces damage with counters, it does not work. However for wither/infect, yes it does work since neither deals damage but deals -1/-1 counters.

I disagree. It deals damage in the form of -1/-1 counters according to reminder text. So I would say it is prevented.

Well, that is quite the conundrum.

I'm going to have to go with that the Phytohydra receives -1/-1 counters and the damage isn't prevented.

2gk is right! Phytohydras ability replaces damage! Infect changes the result of damage! Phytohydras ability will replace infect damage in the same way that it replaces any other damage, and itll get +1/+1 counters! The damage doesn't have the chance to put -1/-1 counters on Phytohydra since it's never actually dealt!

Yeah it can't get the -1/-1 counters because they are a replacement effect for the damage dealing, but it is still prevented when it's first dealt.
