Graft Brion with the equipment then when you cast the Dreadnought, have Brion throw it before its ability triggers. 12 poison much?

Isn't equipment placed on a creature as a sorcery?

I guess, equipping the giant happened the turn before.

Or the same turn, just equip, then cast the Dreadnought...

I was confused because I didn't think you could interrupt the come into play ability.  In other words, if you active an ability when it comes into play aren't you effectively activating an ability before it is in play?

For instance, once it hits the table you need to make a choice.  Sacrifice 12 creatures/Sacrifice the card.  Then you make that selection, which is sacrifice the creature and then you activate an ability that says sacrifice this creature.  There can't be 2 sacrifices going on, right?  Sometimes it is in the wording on the card and then sometimes they just make rules against it happening.

There also used to be a rule that says if a creature is getting sacrificed or on the way to the graveyard you couldn't sacrifice it to activate an ability and that may have changed since I played.

Nothings changed once saced a creature can't be sacced again. Nor can it be used to pay for a different activated ability.

This works because the dreadnought has a trigger ability when it comes into play it goes on the stack in response sac the dread to stoutarm. Dreads abilty then resolves asking you to choose. Choose to sac the dread you go to do so find the dread is not there. So the abilty does nothing.

You might be getting mixed up because WOTC at one point changed the ability on Phyrexian Dreadnought to be a replacement effect instead of a triggered ability. However they undid the errata in a later rules update, so it now plays like it reads.

IE: Phyrexian Dreadnought comes into play.
'Sacrifice 12 power of guys or I die' triggers and goes on the stack.
While it's on the stack, Brion chucks the Dreadnought.
Your opponent dies due to having 12 poison counters so the trigger never resolves (unless you're in multiplayer or they play damage prevention or similar)
If the Dreadnought's trigger resolves, choose not to sacrifice anything.
