I like it! 

Since they are both Enchantments they could both be pulled back as well. 

Why does Form have to be so darn expensive!  It just seems like someone sneezed and they made the mistake of thinking it was bigger when it should be a cheap enchantment. 

I mean, 1 lava axe and it's game over!

Is there a blue or red version of the Shaman?

Form isn't the only way to get to five life. Use Channel or something.
