Equip Blade of the Bloodchief on Triskelavus then pay 1 mana to puts a token. Sacrifice token to deal 1 damage and trigger of BoB on stack to put a +1/+1 counter on Triskelavus.

I thought tokens didn't go to the graveyard?

Tokens hit the graveyard, then disappear (or however the technical term is for exile).  So they will set off "go to the graveyard" effects.

For flavor, you could add in Captivating Vampire to turn the Triskelavus into a vampire and he'll be rather big, rather quickly! 

I also wondered why Ashnod's Alter isn't in there for infinite damage.

If you're particularly insane Mephidross Vampire.

Oh ok.  radical.  I just got a Triskelavus and i was thinking about something similar, but i wasn't sure if it would work.
