well, if you are up to 7 islands you can pull it off with 1 of each, but with 2 high tide, you only need 4 lands...

with 7 islands:      you cast high tide so your islands give you 2 blue mana instead of 1, then tap all your lands to stack mana,   cast palinchron and untap all your lands. 5 mana left so you pay 4 to get palinchron in your hand... 1 mana gained... do it as much as you like to get enough spell played this turn and cast brain freeze   deck the opponent so at his turn he cant draw and loses the game  ,  of just make him draw with a stroke of genius or something... 

but the combo is for the brain freeze

With Medallions? yup turn 4...but without them maybe turn 6 or 7...

Turn six w/o the meds.

I played a variant of this using laquatus instead of brainfreeze, though I must say braingeyser works just about the same.
