Play Storm World to psych out your opponent, then play Cursed Rack and Gnat Miser (in that order). That'll lower your opponent's maximum hand size to three, so they'll take damage every turn unless they can prevent it every turn.

the max hand size starts out at 7 so wouldn't it go down to six after you play the rat?  the rack just says they have to get discard cards down to four, it says nothing about max hand size.

As Cursed Rack enters the battlefield, choose an opponent.
The chosen player's maximum hand size is four.

...and since it goes in "timestamp" order, if you play Cursed Rack and then Gnat Miser, it lowers the opponent's hand size to four, and then one less to three.
