Whenever a spell or ability causes its controller to shuffle his or her library, that player puts a card from his or her hand on top of his or her library.

This I believe is Commander 2013's rendition of Stranglehold, which was broken with Ral Zarek and Temporal Aperture. I'm looking at this and I know that there is at the very least a Library Lock hidden somewhere but I can't pinpoint it.

What I have found:
This caters to Future Sight, Vampire Nocturnus, Lost in the Woods, Delver of Secrets, and the like.
Gives Miracles a second chance (Myr Mindservant/Madblind Mountain + Thunderous Wrath, or such)

The best I've come up with so far is: Widespread Panic, Whirlpool Rider, Lifeline, Bottled Cloister, another creature.
Perhaps: Widespread Panic, Lantern of Insight, Myr Retriever, Lifeline, Etherium Astrolabe?

Since the rules for timing do not allow casting sorceries during the draw step, you may activate the soldier of fortune (see 504.3 below) during the draw step after your opponent has drawn the card. He cannot cast anything, he only may activate abilities like cycling.

307.1. A player who has priority may cast a sorcery card from his or her hand during a main phase of his or her turn when the stack is empty. Casting a sorcery as a spell uses the stack. (See rule 601, “Casting Spells.”)

504. Draw Step
504.1. First, the active player draws a card. This turn-based action doesn’t use the stack.
504.2. Second, any abilities that trigger at the beginning of the draw step and any other abilities that have triggered go on the stack.
504.3. Third, the active player gets priority. Players may cast spells and activate abilities.

I would say that enchantment from New Phyrexia block that lets you exile one of their top two whenever they shuffle. You can make them lose whatever they put on top if its good, or keep it there and throw out the other card if its bad.

You could just see the despair in their eyes as Widespread Panic takes root and spreads like the plague...

Well, Maralen doesn't work, since Widespread Panic only triggers when something THEY control makes them shuffle. So the trick would be to give them Maralen, maybe?

Psychic Surgery does have good synergy, but you are right, even thought Soldier of Fortune was the exact card I was looking for, it does not work due to the shuffler needing to be in control of the shuffling spell or ability.
I'm not sure I'd grant them a Maralen of the Moonsong, at it allows a method of self destruction searching.

So what about adding Stranglehold and Bazaar Trader to the mix. Stranglehold shuts them out of searching, but doesn't stop the mandatory shuffle part of Maralen's ability. So you stop them from drawing, stop them from searching, then force their entire hand back on top of their library.

Donating the Maralen is a good suggestion.

Add hive mind, have a lot of shuffle effects, and keep this out. Seems best in grixis colors to me. You could do a notion thief kind of deal, or use sire of insanity to keep opponents without cards aside from drawing additional ones if they're lucky enough to. You could also use fatigue to skip draw steps

What I'd probably do is Possessed Portal, Notion Thief, Hive Mind, Widespread Panic, and Sire of Insanity to make it as difficult as possible to answer. And I'd play bojuka bog and bounce lands to exile graveyards or something. If it were 5 colors, I'd use rest in peace.
